This special scenario exists when the physical therapy plan of care comes to the last visit, the initially planned discharge visit date. However, at this time and in your opinion, the patient is not appropriate or ready for discharge due to the patient not responding to the PT interventions as expected and/or there has been an unanticipated change in the patient’s status. So your plan is to extend care, by documenting a reassessment (reexamination) to request additional skilled home PT visits.
